Patient Forms

All new patient forms are to be filled out in-office. If you are scheduled for a new patient appointment, kindly arrive 10-15 minutes earlier to complete the required intake paperwork.

Payment Options

For your convenience, we accept cash, personal checks, debit cards, and most major credit cards, including CareCredit. Payment is expected at the time services are performed.  Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
